A transport company has been fined $320,000 after a truck driver was killed by a B-double at a western Melbourne depot.
The man had been standing on a pile of wood debris about 10 metres away from a reversing truck when he either slipped or tripped and was run over.
He died at the Yarraville depot.
A WorkSafe investigation found transport company Malec Holdings should have reduced the risk of injury or death by removing the need for B-doubles to reverse in the workplace.
It should have also implemented a traffic management plan that identified safety hazards and risks.
A County Court judge on Thursday convicted and fined the company $320,000 over the death in October 2020.
WorkSafe executive director Narelle Beer said workplace tragedies were far too common.
"This awful death could have easily been avoided if the right controls were in place," she said.
"In any workplace where vehicles are present, a traffic management plan is an essential measure to prevent loss of life or serious injury."
